The Creeps' Photo is a group photo of the Creeps posing with PUPPY to commemorate his leadership of the gang. It includes a number of signatures, though most are illegible.

Upon exiting the Creep's Parking Garage, PUPPY is ambushed by three Creeps who seem like they're about to attack him only to present him with the photo and leave.

• Curiously, the photo includes both PUPPY and Nerve and was signed by Donvius despite both Nerve and Donvius having been killed during PUPPY's assault on the parking garage.
• The Creeps' Photo is the only item in the game to not have a practical use in any way.
